Telehealth Informed Consent

This is the information Heinz Aesthetics LLC gives you before a video visit with Scott Heinz, PA-C. Please read it before you book. How the visit works is also described on the telehealth page.

The provider

Your video visit is with Scott Heinz, PA-C, a physician assistant practicing in Florida under physician supervision.

Supervising physician: Stanley John Mathew, MD, Florida License ME172759.

Florida only

You must be physically located in the State of Florida for the entire video visit. At the start of the visit you will be asked to confirm the state you are in. If you are not in Florida, the visit will not go ahead by video. Scott is not offering telehealth to patients located in other states.

Be in a private place where you can speak freely. Do not join from a moving vehicle if you cannot give the visit your attention.

What the visit is

Visits are conducted by live two-way video only. There is no audio-only fallback. A visit is not a phone-only call, an email, or a text conversation. The visit has the same purpose as an in-person consultation: Scott reviews your history and decides what, if any, care is appropriate.

• You have a video consult with Scott.
• Labs are ordered only if they are needed.
• A prescription is written only if it is clinically appropriate after the evaluation.
• Follow-up visits are scheduled if you start a plan. Follow-ups are by video only while you are in Florida, or in person at the studio.

Benefits

A video visit can save a trip to the studio when an in-person exam is not required. You still see the same provider.

Risks and limits

• Scott cannot listen to your heart, feel a lump, or otherwise examine you by hand. Some problems cannot be assessed safely by video. He may stop the video visit and ask you to come to the studio, or to seek urgent or emergency care.
• The video can freeze, drop, or be hard to hear. If the connection is not good enough for safe care, the visit stops and is rescheduled. It is not continued as an audio-only visit.
• Someone near you, or a device you did not secure, could see or hear the visit. Use a private room and your own device.
• Electronic records can be interrupted. We use systems that are meant to protect health information, and no system is perfect.
• A delay in labs or in a follow-up can delay a decision. Do not wait on a video visit if your symptoms are urgent.

Emergencies

Telehealth is not emergency care. If you may be having an emergency, call 911 or go to the nearest emergency department. After you call 911, you may tell the practice when you are able.

Alternatives

You may be seen in person instead, at 760 Currency Circle, Suite B, Lake Mary, FL 32746. You may also decline care. Choosing a video visit is voluntary.

Prescriptions

This notice is not an advertisement and not an offer to prescribe or ship medication because you saw an ad. Scott will prescribe a medication only if, after evaluating you, he believes it is clinically appropriate and legal. You may not receive a prescription. A listed price is a visit fee or, if a medication is prescribed, the fee for that medication. It is not a promise that the medication will be prescribed.

Some medications cannot be started by video. Controlled substances are limited by Florida law and by federal law, and an in-person evaluation may be required. Scott will follow the law in effect on the day of your visit rather than a general promise on this page.

If a compounded medication is prescribed, it is prepared by a licensed compounding pharmacy. Compounded medications are not FDA-approved. Brand-name trial results do not describe them.

Privacy and records

The visit is documented in your medical record. How that information may be used is in the HIPAA Notice of Privacy Practices. Website and text-message rules are in the Privacy Policy and SMS Terms. You do not have to opt in to texts to have a video visit.

Do not use email or text to send symptoms, photos of your body, or medication questions if you can avoid it. Those channels are not the visit.

Fees

The Free Wellness Consultation has no charge. Any later visit, lab, or medication is priced before you agree to it. You may refuse a recommended service. Website terms, including the 72-hour notice for free or reduced-fee services, are in the Terms of Use.

Withdrawing consent

You may stop a video visit at any time, and you may withdraw consent for future video visits. Withdrawing consent does not erase the record of care already given, and it does not cancel a fee you already agreed to for a service already provided. Tell Scott during the visit, or call (407) 279-1789.

How consent is recorded

Reading this page is how you learn what you are agreeing to. The consent that goes in your chart is separate. Before or at the start of the video visit you will be asked to confirm that you:

• are 18 or older;
• are physically in Florida;
• understand the risks and the alternative of an in-person visit;
• understand that a prescription is not guaranteed;
• know to call 911 in an emergency;
• agree to a video visit with Scott Heinz, PA-C.
